Self-Referral Form: 'I Need To Talk To You'

‘I Need To Talk To You’ is a self-referral form/worksheet has been designed to support children and young people with identifying when they need support. It is a tool for building emotional literacy and allowing insight into thoughts and feelings for the adults who are supporting the children or young person.
How could this resource be used?
- To support teaching emotional literacy and identifying issues surrounding mental health and wellbeing.
- To identify children that may need further support for their mental health.
- To support a child 1:1.
- To support a wider class ethos of talking and normalising mental health and wellbeing.

*in line with the PSHE Association 2021 Statutory Guidance.
- KS1: H19 – to recognise when they need help with feelings; that it is important to ask for help with feelings; and how to ask for it.
H20. strategies to respond to feelings, including intense or conflicting feelings; how to manage and respond to feelings appropriately and proportionately in different situations.
H21. to recognise warning signs about mental health and wellbeing and how to seek support for themselves and others.
