'My Year ... Journey': Transition, Positive Pathway & Reflection Worksheet
‘My Year … Journey’: Transition, Positive Pathway & Reflection Worksheet is an activity that has been created to support children and young people in identifying and reflecting upon their personal achievements and successful moments in the past Academic Year.
This activity explores the different feelings linked to moving year groups and supporting with this transition by allowing them to share information about themselves with their future teacher.

- To support children with reflecting on their past school year.
- To create positive pathways for success by establishing personal achievements and successes!
